Move tile markers out of emulation prevention process. Fix for wavefront + independent tile byte alignment. Fix for encoder crash when tiny slices/entropy slices present. Fix for tile marker insertion logic.

Decoder side fix to fix a CABAC decoder misalignment problem when WaveFrontFlush=1, WaveFrontSynchro=0 and independent tiles are used.

F274 & F275 initial integration (wavefront processing and cabac flush).  Known problems with TILES_DECODER (cannot write slice header tile positions if writing substreams).  Fine Grain Slice tests not completely correct.

- add modules and sort librairies into modules
- improve text on main page

remove all relative include paths (containing "../") due to doxygen inability to
properly parse relative includes

- provide source/Lib include path for all libraries/applications
- adjust include paths to start based on source/Lib
- move TAppCommon to souce/Lib

Note: This commit fixes the paths only for MS vc9 and UNIX makefiles,
updates for vc8 and Xcode will follow

Add method to decode multiple bypass bins

 - TComOutputBitstream only contains logic related to packing bitstreams
 - TComInputBitstream only contains logic related to unpacking bitstreams

Separating the two affairs allows for simpler behavioural constraints
to be identified, greatly simplifying the subsequent commits.

Remove E192 related codes. Add doxygen documentations for the new functions. Change names of variables and functions to follow the CamelCase convention.
