Integration of JVET-O0238: Parameters in PPS or slice headers

The proposal was originally called "Parameters in SPS or slice headers" but the decision at the meeting was to signal these parameters at PPS level instead of SPS level. The implementation is according to the syntax and semantics of JVET-O0238-v3 which corresponds to the meeting decision.
