Fix parsing dependencies in the VUI syntax by moving the
general_progressive_source_flag and general_interlaced_source_flag to
the VUI

- converts static arrays to vectors for subpicture related parameters
- improved encoder parameter checking
- mode automatic calculation of subpicture ID length at encoder
- update theoretical limit of maximum number of subpictures

- initialize slice map from subpicture map
- encoder support for inferring slice map from subpicture map
- improve software manual

- new pattern: create a class to collect all parameters  related to this SEI
- existing code can be refactored to use the same pattern for grouping to avoid endless assignments

Note: fixes executable flags for some unrelated config files

Update the manual to say that entry points for tiles is always present for slices with multiple tiles

Update software manual to remove obsolete parameter Log2MinCuSize and add new parameter Log2MinCuSize

Modified SAO for 12 bit coding.
Also removed SAO offset from the HEVC RExt PPS extension.
The HEVC RExt PPS extension and the associated
cross-component-prediction should be removed in the future

As requested, a description of the -opl option has been added to "doc/software-manual.tex" and the line "m_oplFilename += ".opl" has been removed.
