Add a constraint to require that when vps_independent_layer_flag[ i ] is equal to 0 there shall be at least one value of j such that the value of vps_direct_dependency_flag[ i ][ j ] is equal to 1.

  - Fix cross tile / slice prediction bugs.

- Prevent negative coefficients on the decode side.
- Prevent illegal coefficients outside the range [-128,127] on the encode side.

num_extra_slice_header_bits is not present in the spec, and is not used any more in the slice header

  - Add Access Unit detection to support  multi-layer streams containing multiple pictures per AU.
