This reverts commit 1a33f26e.

includes:
- rename flag HandleCraAsBlaFlag to HandleCraAsCvsStartFlag
- changes of NAL unit types (reduced number)
- profile_tier_level and general_constraint_info structures

- remove duplicate ALF parameters from Slice
- add activation (to avoide overwriting when a new APS arrives)
- rename macro

Attempt to resolve conflicts with ENABLE_QPA_SUB_CTU but finally make it mutually exclusive with JVET_M0113_M0188_QG_SIZE (to be fixed later)

  - Add same fixes for ALF with SIMD disabled.
  - Additional fix for case where a chroma PCM block is not aligned with 4x4 based ALF processing.

TCoeff is not always an int. Using the TCoeff type as template parameter
is preferred.

Obsolete code relying on dead code like g_aiT64 array.

A substantial coding loss is observed in CTC starting at commit
c52f46f0 (!362) which relates to
M0445. Fastest fix is to disable the feature
