- compile fix for macOS
- initialize rectangular slice map before using it for subpictures

move qp derivation into config, use base QP instead, result in SPS QP is controlled by encoder not by user

   - Extend "RefPicIsScaled" logic to cover corner cases where the scale factor is 1:1, but the scaling window offsets are not aligned.
   - Specify scaling window offsets in units of SubWidthC and SubHeightC pixels.
