Fix behaviour of UseIdentityTableForNon420Chroma configuration parameter

When UseIdentityTableForNon420Chroma is set to 1, the chroma QP mapping
table should be the identity table, i.e. QP_chroma = QP_luma. The encoded
table parameters are modified such as to generate the desired identity table.
