Allocate single large buffer instead of many small buffers for
holding scaling lists data

Note: in VVC draft 6 "–	If intra_bdpcm_flag[ ][ ] is equal to 1, ctxInc is set equal to 1.", which was set equal to 4 in VTM5. This integration also implements this change, to align cbf ctx with VVC draft 6.

Clear entire buffer to ensure all values in the buffer are valid
even though some values may lie outside the subset of coefficients
that is effectively coded

There is a conflict with JVET_M0427_INLOOP_RESHAPER that is triggering a debug assertion that checks that the width is not 1.

Fixed transition point between GR and TU + EG(k) escape codes for coefficient coding. Use limited prefix length for escape codes.

Move the gtr2 flag (the abs_level_gt3_flag in text) after the par_level_flag in the first coding pass
