Make RdCost members non-`static` to use correct weighting tables for each layer in multilayer encoding

- remove picture header allocations at the encoder
- use correct boundary location in reference pictures after removal of new PH

Should also fix memory allocation issues with PicHeader.

This class contains a pool of objects that can be used and reused
while minimizing the amount of required memory allocation and
deallocation operations
