Fix for Visual Studio compiler warnings and errors when JVET_R0351_HIGH_BIT_DEPTH_ENABLED is enabled

Move DQ and SDH from picture header to slice header.
Disable combinations of either DQ/SDH with slice_ts_residual_coding_disabled_flag.

This reverts merge request !1475

- LFNST signaling depending on transform skip flags
- disabling chroma LFNST for single tree
- additional scaling condition for LFNST

Bugfix: Use picture header for checking presence of scaling list/matrix instead of SPS when disabling scaling matrix for LFNST.

Enable BDPCM for Chroma for 444.

Add a description of ChromaTS into the software manual.

- The 16x16 coeff. zero-out for DST-7/DCT-8 is made disabled in VTM when MTS is turned off according to the suggested fix in #447.
- This fix does not change CTC results.
