Fix ticket #267 on wrong SAD calculation in xGetSAD_NxN_SIMD(...) if rcDtParam.subShift is not equal to zero and input size is equal to 4x4.

move m_fixedFilterNum to CommonDef.h, rename bRdo and bRedo to isRdo and isRedo, remove redundant stat derivation.

- Removed lfnst_mode context modelling dependency on intra-prediction mode.
- Second bin of lfnst_mode is bypass-coded rather than context-coded.

  - Fix to use unfiltered reference samples for RDPCM mode.

  - Fix bugs in N0280 - Transform skip residual coding.

Always using six tap filters for affine blocks no matter whether all CPMVs are the same. Patch provided by minhua.zhou@broadcom.com

- Previous name: Reduced Secondary Transform (RST)
- The parameter, LFNST, should be added in cfg file for enabing LFNST, i.e. LFNST = 1

Changed condition for "fast" RA in reduceHadCandList() and renamed variables using camelCase notation
