JVET-Q0503: encoder only improvement for palette mode, and JVET-Q0712: fix encoder issue at high QPs in the error limit map (g_paletteQuant) used in deriving palette table. Submitting the two software changes together since the fix in JVET-Q0712 is on top of the proposed error limit table in JVET-Q0503.
