JVET-Y0056: Clear MinCR definition for high tier for levels below 4

Changed mainLevelTierInfo which implements Table 136 in clause A.4.2 of the draft text. Added check in EncAppCfg.cpp which tests for the unsupported combination of high tier and levels below 4.
