Remove StreamMergeApp

Karsten Suehring requested to merge ksuehring/VVCSoftware_VTM:remove_merge_app into master

StreamMergeApp has not been maintained for several years. The concept of multiple layers in VVC was extended beyond the scope of the app shortly after the initial commit. Merged streams are unlikely to conform to the spec. So the tool should be removed from the reference software.
