JVET-O0428: (HLS part) Number of bits of referencing LMCS APS IDs in SH from 5 to 2

Change slice_lmcs_aps_id signaling bits from 5 to 2 to align with updates in VVC specification vD. This automatically constrain max LMCS APS number to be 4.
