BDPCM Chroma syntax is disallowed to signal when ACT is enabled for CU.
Decision(BF/SW): Adopt JVET-Q0353 method 1.

   - Extend "RefPicIsScaled" logic to cover corner cases where the scale factor is 1:1, but the scaling window offsets are not aligned.
   - Specify scaling window offsets in units of SubWidthC and SubHeightC pixels.

JVET-P1019: Output layer set

This bugfix is needed for last candidate to have correct refIdx.
Else, it can result in bad merge derivation later.
