Implementation of VUI syntax as documented in VVC D8

The CCALF reconstruction function used by the encoder is separate
from the one used by the decoder. !1280 addressed issues in the
decoder. This commit addresses a similar issue in the encoder.

JVET-Q0503: encoder only improvement (4 items) of palette mode
JVET-Q0712: fix palette encoder issue at high QPs. Fix on top of the optimized table in JVET-Q0503.
